LSU-Sponsored Student Health Insurance Plans

LSU contracts with an insurance provider to offer the best possible benefits for a reasonable premium. All students who do not have health insurance are encouraged to purchase one of the University-sponsored plans, as accidents or illnesses requiring off-campus care could be extremely costly.

Every month, a Student Insurance Representative will be available at the Student Health Center (Room 176) for questions/concerns about GM-Southwest Student Insurance.

Who is eligible?

All full-time and part-time students registered for resident study, excluding individuals registered for or in any of the courses/activities offered through the Continuing Education programs of the University, are eligible to purchase the plan for themselves and their dependents.

Please note: Non-immigrant international students are required to have adequate insurance. Therefore, all F and J non-immigrant international students will be automatically billed for the Basic Plan every semester (including repatriation and medical evacuation coverage) on their fee bill, unless proof of other adequate insurance is presented to the ISO and a waiver is signed prior to the ISO semester deadline. How to enroll:

Students can enroll themselves through PAWS (“additional services” section) through the 14th class day of that semester. Students wanting to 1) enroll for coverage after the 14th class day 2) enroll in the 3-pay or 9-pay plans or 3) purchase coverage for dependents, must do so directly with the insurance company at www.LSUstudentinsurance.com. There are no deadline dates to purchase coverage during the semester, however please note that premium amounts will not be prorated.

PAWS Insurance Codes:

Coverage selected through PAWS for any fall semester will be automatically reassessed on the following spring semester fee bill. If your class schedule is purged for any reason, you must ensure the insurance is reassessed on the subsequent Bursar Office bill. Dependent coverage will not be automatically reassessed; it must be renewed manually.

Graduate Assistants holding a full-time (50% effort) assistantship and eligible to receive the Graduate Assistant insurance premium subsidy must enroll for one of the plans via PAWS on a per-semester basis to receive the premium assistance. There will be two fee bill entries, one for the full insurance charge and one for the amount credited due to the subsidy.

It is extremely important that you read your plan brochure to understand the specific terms and conditions, benefits, and coverage limitations that are described therein. Brochures are also available at the SHC, the ISO, and at www.LSUstudentinsurance.com. The plans have cost containment features to keep your health costs as low as possible while maximizing your premium dollar and conserving your benefits, as appropriate.


1. Electronic referrals are required for all care rendered outside of the SHC, unless the care meets the plan’s definition of emergency or coordinated by the 24/7 Nurseline when the SHC is closed. Please contact the SHC for hours of operation by calling 225/578-6271. If you call the SHC outside of normal hours of operation, you will be given the phone number to the 24/7 Nurseline so that you may call to speak with a registered nurse.

2. All outpatient surgeries, inpatient stays, therapies, durable medical equipment and inpatient mental health/substance abuse treatment require pre-authorizations. Determinations can be made by calling 1-866-751-2723.

3. Preferred Provider Organization (PPO): these are physicians, hospitals, and other medical providers in Louisiana and throughout the United States that the insurance company has contracted with to provide services at lower than normal costs. While you may request a referral to any medical provider, the best reimbursement rates will be from the Verity Health Network of contracted providers. A copy of the primary network (Verity HealthNet) and secondary network (BestCare/Beech Street) PPO lists may be obtained by going to: www.verityhealth.com or www.bestcareinc.net .

4. As an additional service to the student, you may request a “pre-determination of cost” form during your request for pre-authorization. This form will provide you, as a consumer, with high level cost information on those facilities in and out of the contracted PPO. The intent of this form is to allow you the opportunity to access your potential out-of-pocket cost under the various settings.


Students and their dependents also have the ability to select stand-alone Vision and Dental programs. Please note that coverage under one of the SHI plans is not required to purchase the Vision or Dental products. Enrollment in the Vision and Dental programs can be handled either through PAWS by selecting the appropriate Additional Service code (see code table below) or online at www.LSUstudentinsurance.com.
