General Information and Claim Filing

The Student Health Center on campus offers quality medical care at a very low cost, and is available to all registered students and their spouses. It is not necessary that a student have insurance to use the facility's services.

For students that do have health insurance coverage, the SHC has an Insurance Billing Office to assist those students with questions and the submitting of itemized bills/health insurance claims for charged services rendered at the SHC. This office is located in Room 176 on the main floor of the SHC building. The office can be reached by dialing 225/578-3304, faxing information to 225/578-0805, or sending an email inquiry to shcinsurance@lsu.edu.

Insurance claims must be requested at the conclusion of every clinic and/or pharmacy visit by completing and signing the SHC Insurance Billing Release of Information form at the Cashier Desk. The form allows our patients to request billing to the Student Health Insurance, an off-campus private insurance (must present that company's insurance card) or to another individual.

Please note: due to strict HIPAA and FERPA privacy laws, only the patient’s signature can be accepted on the release form. Parents, guardians, spouses, or any other relatives or friends cannot authorize the release of any medical or personally identifiable information on a patient. Signatures will be compared to those in the patient’s medical chart.
